Medicare Tobacco Cessation Follows Lead of Many Private Insurers

The Centers for Medicare and Medicaid (CMS) announced March 23 that it will begin covering the cost for tobacco cessation for some beneficiaries, following the lead of many private insurers who know that reducing risks can save both lives and dollars.

The Centers for Medicare and Medicaid (CMS) announced March 23 that it will begin covering the cost for tobacco cessation for some beneficiaries, following the lead of many private insurers who know that reducing risks can save both lives and dollars.

The Centers for Disease Control and Prevention (CDC) has estimated that 9.3 percent of Americans age 65 and older smoke cigarettes. About 440,000 people die annually from smoking related disease, with 300,000 of those deaths in those 65 and older.

Beyond the benefit of better health through tobacco cessation, the new move may also help Medicare cut through the issue of rising costs. Researchers estimate that smoking accounts for about 10 percent of the total costs of the Medicare program,

"Covering smoking and tobacco use cessation counseling for seniors has great potential to save and improve lives for millions of seniors,” said CMS Administrator Mark B. McClellan, MD, PhD, as he announced the new benefit. “This is another step in turning Medicare into a prevention-oriented health program."

The new benefit is available only to those who already have an illness caused or complicated by tobacco use, including heart disease, cerebrovascular disease, lung disease, weak bones, blood clots, and cataracts – the diseases that account for the bulk of Medicare spending today. It also applies to beneficiaries who take any of the many medications whose effectiveness is complicated by tobacco use--including insulin and medicines for high blood pressure, blood clots and depression.

Medicare has added several new significant preventive benefits in 2005: cardiovascular screening blood tests and diabetes screening tests for all Medicare beneficiaries and the “Welcome to Medicare” prevention physical for new Medicare Part B enrollees. CMS has also developed a guide for preventive services for physicians, providers, suppliers and other health care professionals and they have established a preventive services website.

MedicareÂ’s new focus on prevention follows an overall trend by private insurers to adopt health management strategies to both improve enrollee health and lower costs. According to a study release March 18 by the National Business Group on Health and Watson Wyatt, the number of employers adopting lifestyle behavior change through a health plan doubled to 40 percent this year.

“Increasingly, both private and public insurers are recognizing that reducing health risk behavior makes sense,” Lehmann said. “Reducing the risk for disease is the logical next step in changing health care from a system that just treats disease to one that promotes a healthier lifestyle.”

Scientists, educators, land managers, aquatic specialists and public policy officials are gathering in the nation's capital February 22-27 for the 10th annual National Invasive Weeds Awareness Week (NIWAW) - a series of meetings focused on the devastating impact of invasive weeds on the environment and economy.

Land managers, aquatic specialists, scientists, educators and public policy officials are gathering in the nation's capital February 22-27 for the 10th annual National Invasive Weeds Awareness Week (NIWAW) - a series of meetings focused on educating our federal policy makers and elected officials about the devastating impact of invasive weeds on the environment and economy.

Across the nation, the most significant invasive weeds are spreading at approximately 15% per year. This rate of spread will result in a doubling of infested acres in less than five years. According to a recent Cornell University report, the economic impact of invasive plants and weeds in the U. S. is estimated at $34.7 billion annually.

National Invasive Weeds Awareness Week is coordinated by the Invasive Weeds Awareness Coalition, a broad-based group of public and private stakeholders concerned with the prevention and management of noxious and invasive weeds.

A highlight during this year's NIWAW events will be the first official meeting of the Healthy Habitats Coalition (HHC). Current federal efforts and budgets are inadequate to address the invasive species crisis effectively, and local and state interests alone cannot resolve invasive issues. The Healthy Habitats Coalition will provide training for concerned citizens and advocate for legislation and policies year-round that improve the prevention and management of invasive and noxious weeds, as well as other invasive pests and diseases.

"Invasive weeds can impair wildlife and fish habitats, reduce the diversity of our natural resources and dramatically reduce crop yields," said Lee Van Wychen, director of science policy for the Weed Science Society of America. "National Invasive Weeds Awareness Week gives land managers, scientists and educators a chance to share their knowledge and concerns with public policy makers so we can slow the spread of invasive weeds."

Problems in the Bedroom Can Indicate Heart Problems Particularly for African American and Hispanic Men

Erectile dysfunction (ED) is often the first and earliest sign of a more significant cardiovascular condition, according to a study in The Journal of Sexual Medicine. The study presents results from the Minority Health Institute (MHI) Expert Advisory Panel.

“Erectile dysfunction is not just a quality of life issue, but needs to be considered a significant public health concern associated with preventive cardiovascular medicine,” states lead researcher and author of the article, Kevin Billups, MD. “The earlier a man is evaluated for ED, the better the outcome for maintaining good erectile function and good cardiovascular health.”

Research suggests that medical evaluation for ED in patients needs to be integrated into an evaluation for cardiovascular risk as well. The study stresses that early recognition of ED, “particularly in high-risk and underserved minority populations” like African Americans and Hispanics, can lead to early diagnosis of previously undetected cardiovascular risk factors and subclinical vascular disease. African American men are 20% more likely to have ED than Caucasians, according the National Health and Social Life Survey.

While ED has traditionally been seen as a secondary complication to heart disease, diabetes, and other vascular illnesses, key findings in various clinical research studies have shown that ED was often present prior to the diagnosis of these other medical conditions. Because of this strong association, ED is now being considered an important barometer of a manÂ’s overall cardiovascular health and an early symptom of a more generalized vascular problem.

Adopting the ideas presented in this paper could have a significant impact on preventive cardiovascular health strategies,” adds Dr. Billups.

The MHI recommends that all men aged 25 years and older should be asked about ED. Any signs of the problem should prompt physicians to aggressively assess whether or not the patient is at risk for cardiovascular disease.

On May 25th, President Bush signed the Small Business and Work Opportunity Tax Act into law. For the second time in just two years, the Kiddie Tax, introduced as part of the massive Tax Reform Act of 1986, was made even broader.

Prior to 2006, any unearned income above a certain threshold earned by a child under the age of 14 was taxed at the parent's tax rate. In 2006, the Tax Increase Prevention and Reconciliation Act of 2006 expanded the Kiddie Tax to include children 17 or younger who earn more than $1,700 (in 2007) in interest, dividends, capital gains, and other non-wage income.

This year, as part of the Small Business and Work Opportunity Tax Act signed into law on May 25th, the Kiddie Tax was bumped up to age 18, and up to age 23 for children who are full time students, for a child whose earned income does not exceed more than half of that year's support, effective 2008.

"Understanding how your child will be taxed is very important when determining how to best save money for their college education," explains Andrew Schwartz CPA, founder of FindAGoodCPA. com, a site where taxpayers can locate a CPA in their state who specializes in their specific tax issues. "Now parents who managed to build up an education nest egg might be hit with higher tax bills throughout their children's undergraduate years."

For 2007, the first $850 of net investment income earned by a child isn't taxed, and the next $850 is taxed at a rate of either 5% or 10%, depending on the type of income earned. Any additional income is taxed based on the child's age as follows:

Children under the age of 19: A child's net investment income earned during the year that exceeds the $1,700 threshold (in 2007) is generally taxed at the parent's marginal tax rate.

Children between the age of 19 and 23: If a child is a full-time student whose earned income does not exceed more than half of their support, a child is taxed under the Kiddie Tax rules.

Children over the age of 24: Upon reaching the age of 24, a child's income is taxed using the same tables that apply to single adults. For 2007, the first $7,825 of net taxable income is taxed at 10%, and then the next $24,025 is taxed at 15%. As with all taxpayers, corporate dividends and long-term capital gains are taxed at a lower rate.

"Now that the Kiddie Tax applies to children through age 23, it makes even more sense for parents to consider saving for a child's college education either in their own name or within a 529 Plan or Coverdell Education Savings Account (ESA)," says Schwartz.

"There are other variables to factor in as well. Investments made in a child's name tend to reduce the amount of financial aid available to pay for college. Plus, if a child receives a full scholarship or decides not to go to college, any money saved in that child's name becomes his or her property upon reaching the applicable age of majority in their home state. And don't forget that the Pension Protection Act of 2006 made tax-free distributions from 529 plans permanent."

Every election year, pundits and doomsayers proclaim that political powerhouses are using voter apathy as a tool with which to make presidential elections “a private party to which most American voters are not invited.” This assertion is clearly designed to ruffle the feathers of the “unwashed masses” and prod them to the nearest voting booth come November.

However, these opinion-shapers are also overlooking the obvious fact that major political parties really don’t have to actively work to lower the interest level of most voters. Today, people don’t follow the news – especially political and world events news -- as closely as they once did. This is despite the fact that more information is out there than ever before (due, in no small part, to Web sites and blogs.)

But does the general public not care as much about politics today because theyÂ’re out of touch with the news? Or do we avoid traditional news outlets because weÂ’re better informed about media bias -- and more aware about how they tailor coverage to attract advertisers?

Traditionally, voting has been seen as a privilege of citizenship as well as a civic responsibility. Today, itÂ’s lost its luster and this constitutional right is considered more as a choreÂ… something to try and fit in between the full-time job and the kidsÂ’ soccer practice.

Once I’m at my local voting facilities, am I truly empowered? Does my one vote – out of more than 100 million – really make a difference? When it comes down to it, the act of pushing a button (or touching a screen -- or filling in/punching out an oval box --or pulling a lever) only means that I’m canceling out the vote of someone who has different political opinions and leanings than mine.

Worse yet, the voting machine can’t distinguish between a well-researched vote and someone playing “connect the dots.” I may meet my civic obligation with the noblest intent, but tell that to the person in the next booth who may have sold his/her vote to the highest bidder.

And with an elaborate system of checks and balances (never mind the “old boy network”) at play, how much freedom and power does the Chief Exec really wield? In the end, how different will a Candidate A”-run country be than a “Candidate B”-run one? (And, once elected, will our head “civil servant” serve the needs of the average citizen -- or the corporate cronies who lined the war chest to get him elected?)

So, why bother? Why not just bow out and leave the decision-making solely to the people who care passionately about the results?

Because, at the heart of it, weÂ’re all a community. I may not feel an overwhelming obligation or affiliation to a major political party - or to its candidate of the moment. And I certainly canÂ’t speak eloquently and intelligently about todayÂ’s most pressing issues (or about any political issues, for that matter.)

But what about my loyalty to the woman down the street, the one who not only provides afternoon snacks to the neighborhood kids, but who also fervently waves a sign outside my ward for hours at a time -- in a cold New England winter, no less – because she still believes in the political process?

Even if I donÂ’t necessarily support her candidate, we both ultimately want the same thing. We both want a safe, peaceful, honest place to call home, one with good education and quality health care. Somewhere where families arenÂ’t taxed beyond their means, and where corporations pay their fair share. A place where opportunities abound, where jobs remain available, and salaries are competitive. A land where no one should have to go to bed hungry or freezing. A country with clean air, water, and land; one that we can turn over to our children in better shape than we inherited it from our parents.

In the end, we both want a wise, intelligent leader (with integrity to spare) to get us to that place – no matter what his particular party affiliation. And both our votes will help get him elected.

So this November, IÂ’ll trudge reluctantly to the polls and vote for the lesser of two evils. IÂ’ll borrow the optimism of the people around me and smile like I really want to be there. And IÂ’ll try to delude myself that, just this time, my vote will really make a difference.
